How old should a kitten be before separating it from its mother?

Our cat is a 5-month-old American Shorthair female eating Purina Kitten Chow. What is an ideal weight for her at this time?

We found a kitten in the marsh behind our house and the vet said it was about 4 weeks old. Usually a kitten has its mother at this early stage. How can we teach it how to clean itself and other things the mother teaches the kitten?
